tutorial for cloud to image conversion

The pcl::toROSMsg functions for going from dense cloud to an image
message now have a tutorial.


The impetus for these are that you may subscribe to a single message
from the kinect (points2) and get the image directly from cloud.

Also, the tools were moved from pcl to pcl_ros, while the functions
remain in pcl.
